Parents/caregivers, please begin to collect the information below to begin the process. Specific documentation is required to register your student. Be sure to have the following:

• Proof of Age
• Proof of Residency (2)
• Physical Form
• Immunization Record
• Emergency Contact Information
• Special Education Information (if applicable)
• Guardianship Information (if applicable)

Physical Form & Immunization Records:

Students Entering School for the First Time:
Provide a copy of your child's most recent physical. Please be sure to work with your primary physician to provide the correct immunizations listed below. State law requires that a complete record of the following immunizations are provided during school registration:

• 4 doses of tetanus, diphtheria, and acellular pertussis (1 dose on or after the 4th birthday)*
• 4 doses of polio (4th dose on or after 4th birthday and at least 6 months after previous dose given)**
• 2 doses of measles, mumps, rubella***
• 3 doses of hepatitis B
• 2 doses of varicella (chickenpox) or evidence of immunity

*Usually given as DTP or Tdap or if medically advisable, OT or Td
**A fourth dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose
***Usually given as MMR

All required documentation should be scanned and saved to your computer for easy access during online registration. Tutorials on how to scan your documents from a mobile device can be found below in the section titled "Technical Assistance and Information." 

Online registration begins Thursday, February 8, 2024. The forms are now open. 

Parents/caregivers will complete the AGSD Kindergarten Pre-Registration form to provide AGSD with their student's personal information. 

If a student is eligible to attend kindergarten, then the Kindergarten Pre-Registration form will direct parents/caregivers to a link to register their student.

Registration is completed through Powerschool, a web-based student information system. Please do not begin this process until you have all required documents from step one scanned, appropriately named, and saved to your computer. Google Chrome is the recommended browser for using the registration system.
