1. State Police Criminal History Report 

• Visit: https://epatch.pa.gov/home
• Click “New Volunteer Record Check” and complete the online form.
• When the record check is complete, you will receive a certification form to submit to AGSD.

For volunteer purposes, there is no charge for a PA Criminal Record Check. 

To be accepted by AGSD, the issue date of the clearance must be within one year of the date of submission to AGSD.

• Visit the Child Welfare Portal: https://www.compass.state.pa.us/cwis/public/home
• Create an account and complete the online process. 
• When the results are complete, submit your documentation to AGSD.

For volunteer purposes, there is no charge for a Child Abuse Clearance unless you previously requested this clearance within the past 57 months.

To be accepted by AGSD, the issue date of the clearance must be within one year of the date of submission to AGSD.

3. FBI Federal Criminal History Report (Act 114) 

• If you have lived in PA for less than 10 years continuously, then you must obtain and submit a Pennsylvania state issued FBI Federal Criminal History/Report in order to volunteer in Avon Grove School District. Clearance must have an issue date within one year of the date of submission to AGSD.
• If you have lived in Pennsylvania continuously for 10 years or more, you may submit a Request for Waiver of FBI Federal Criminal History Report form instead of the clearance. 
• If you already obtained an FBI Federal Criminal History Report, while residing in Pennsylvania continuously within the past 10 years, then submit a copy of the report to AGSD. 

Fingerprinting Process:

• Visit https://uenroll.identogo.com
• Enter volunteer service code 1KG6ZJ to schedule an appointment for fingerprinting. 
• Applicants are charged a $22.60 fee for fingerprinting.

• Applicants may choose their fingerprinting site. 
• Applicants must bring a photo ID to the fingerprinting location. 
• Report Access - AGSD no longer has access to applicants UEID results. Applicants must provide official results. 

4. PDE 6004 Arrest/Conviction Report and Certification Form - Self Reporting

• Complete the Arrest/Conviction Report and Certification form and submit to AGSD.

5. Volunteer Acknowledgement of Policy No.916 Volunteers and Policy No. 806 Child Abuse

• Please read Board Policy #916 & Board Policy #806.
• Complete the Volunteer Acknowledgment of Policy No. 916/Volunteers and Policy No.806/Child Abuse form and submit to AGSD.
