2009-2010 SCHOOL YEAR SPORTS INFORMATION
Tryouts for FALL 2009 SPORTS begin on Monday, August 17th.
|
- It is strongly recommended that fall athletes call their doctor NOW to schedule their June/July sports physical.
|
- Sports physicals for the 2009-2010 school year must be performed after June 1st, 2009 according to PIAA rules & regulations.
|
- All paperwork to be cleared for Fall Sport tryouts is due August 7, 2009.
|
- Paperwork cannot be dated prior to June 1, 2009.
|
- Paperwork will be accepted by the Main Office after June 1, 2009.
|
- All student athletes must turn in a NEW Athletic Information & PIAA Medical Packet for the 2009-2010 school year.

| Individual game admission fees apply for all evening sporting events in the Stadium and the Main Gymnasium. |

PIAA Sports Pre-Participation Requirements |
2009-2010 PIAA RULES & REGULATIONS
Each student athlete must turn the following into the Athletic Office before participation
in any practice or games for any interscholastic sport or on any cheerleading squad:
Turn these in every school year-
- AGSD Athlete Information
- Section 1-Emergency Information-including medical insurance information
- Section 2- Signed parent/guardian consent and parent signed PIAA Rules & Regulations
- Section 3- Medical History
- Section 4- Comprehensive Initial Pre-Participation Physical
Evaluation and Certification of Physician (CIPPE)- sports physical
examination performed after June 1, 2009.
Turn these sections in for subsequent sport seasons AFTER turning in a valid CIPPE:
- Section 5-Parent Re-Certification form-required
before every subsequent season. Signed no earlier than six
weeks prior to start of season. (Not required for fall
sports)
- Section 6-Physician Re-Certification form-Athletic Trainer will determine if this form is necessary based Section 5 information.
